A Clinical Study on the Peripheral Blood Circulation during Extracorporeal Circulation for Open Heart Surgery.


Abstract
In order to estimate the state of peripheral blood circulation during extracorporeal circulation with Sarn¢¥s Heart-Lung-Machine Model 5000 in open heart surgery, we measured the blood lactic acid level and urinary excretion in 32 patients. The results were as follows:
1. The blood lactic acid level at 10 and 120 minutes after the start of extracorporeal circulation was 90.79¡¾35.22 mg/dl and 134.30¡¾40.74 mg/dl respectively.
2. The blood lactic acid level was lower in the group of patients in whom the injec-tion of sodium bicarbonate was required than the group the injection of sodium bicarbonate was not needed. Sodium bicarbonate had been injected due to a decrease of HCO3 in the blood during extracorporeal circulation. These data sug-gest that increased blood lactic acid level does not influence the development of metabolic acidosis.
3. The urine excretion during extracorporeal circulation was maintained over 100 ml/hr., suggesting adequate renal blood flow.
